• Liability Form and Permission Slip

  • Children's Ministry Liability Form (Completed Once per School Year)
  • 3/29 Permission Slip to Xavier HS for All Rise Performance
• A field trip is planned for the 4th and 5th graders to attend "All Rise"
  the Northeast Wisconsin Passion Play at Xavier High School at 6:30pm
  on Wednesday, March 29th (a regularly scheduled class night). A liability form and permission slip will be sent home on March 1st. This will need to be completed and on file prior to the day o the event.
  
  EVENING SCHEDULE 3/29/23:
  5:00pm Student drop off (Dinner provided during regular class time)
  5:45pm Load bus and transport to Xavier HS
  8:45pm Bus returns to Kimberly Church/Student pick up
