Because Baptism is the primordial and foundational sacrament to the Christian faith, many Catholic schools, parishes, priests, or directors of religious programs will need to see a recently issued baptismal certificate in order to make sure that the child or candidate is ready for the reception of additional sacraments. Additionally, individuals who have been asked to serve as Godparent may be required to provide proof of their reception of the Sacrament of Confirmation.

The form below is used for the request of sacramental records at Holy Spirit Parish formerly known as Holy Angels of Darboy, WI and Holy Name of Jesus in Kimberly, WI. All requests are only provided to the individual named on the Sacramental Record or by the parents named on the record if the individual is a minor. To receive a personal copy of the record, an Authorization to Release Information Form must be completed beforehand.

Records requested directly by a parish or diocese do not need written authorization, and will instead be sent directly to the requesting party's address as indicated on the form below.
